The Transition Project
The vision of the Transition Project is to see children and young people being all they can be.
The project is a targeted programme working in partnership with primary schools. It’s mission is to support vulnerable children to make the best possible transition to secondary school.
Our hopes
The aim is to work in partnerships with local schools to build positive and meaningful relationships with children and young people that make a positive difference in the life of the child now and for their future.
The Transition Project is designed to help pupils make a positive move to secondary school, by targeting those who most need extra support through 1:1 mentoring aiming to build self-confidence, social skills, resilience to change and raising aspirations. In addition to the mentoring programme, we offer a year 6 assembly about transition, offer each year 6 pupil the 'It's Your Move' handbook, and run the CAP Kids Money Course over two 2 hour sessions for year 6s.
